The protocol is the one DNB document with no second chance built into the calendar. NBEMS[2] counts the window from your date of joining, the penalty for missing it is stated in the guidelines, and the acknowledgement number it generates is the key to submitting the thesis two years later. This guide walks the process in the order it happens, taking each rule from the NBEMS Guidelines for Thesis Protocol and Thesis Submission and from the notices NBEMS has posted on its Thesis Online portal. One honesty note first: the portal is behind an OTP login tied to your registration number, so what is described here is what the guidelines require and what is visible on the portal before login, not a tour of the logged-in screens.
Step 1: fix your dates on day one
Section IV of the guidelines sets the window. Three-year broad-specialty and super-specialty trainees submit the protocol within 180 days of the date of joining and start the research study on the 181st day. Two-year post-diploma trainees have 90 days and start on the 91st. The clock runs from the date NBEMS holds as your joining date, so confirm that date with your DNB coordinator in your first week; a day's disagreement at the end is a day you do not have.
Write the date down and work backwards from it, because the protocol has to pass through your guide, your department and your hospital's ethics committee before the hospital submits it, and committees meet on their own calendar. Our free calculator at /tools/dnb-deadlines gives the protocol, research-completion and thesis dates from the joining date; an ethics committee that meets monthly means the protocol should reach it with two meetings to spare.
Step 2: write to the word limits
Section III restricts the protocol to fixed limits: a title of 120 characters including spaces, a structured synopsis of 250 to 300 words, an introduction of 300 to 500, a review of literature of 800 to 1,000, aim and objectives up to 200, material and methods of 1,200 to 1,600, and 10 to 25 references in ICMJE[4] style. Section II asks for language that is clear, concise, precise and consistent, without excessive adjectives or long sentences and without redundancy.
Section VII, which describes the thesis layout, is the best guide to what the protocol's methods section should already contain, because the thesis methods are the protocol methods carried out. It names ten elements: study setting, study duration, study design, study sample with inclusion and exclusion criteria and method of selection, intervention if any, data collection, primary and secondary outcome measures, sample size, data management and statistical analysis, and ethical issues including clearance, consent and trial registration. The same section asks for a title that carries the PICO elements and objectives that state participants, intervention, evaluation and design. Our sample size calculator at /tools/sample-size writes the formula, the assumed values and their source into a sentence you can paste into the methods.
Attach the study proforma, the participant information sheet and the informed consent form. The guidelines list them among the thesis appendices, and an ethics committee cannot review a study whose consent form it has not seen.
Step 3: your guide, your department, your hospital
The guidelines place the primary responsibility for ethical conduct, including plagiarism and confidentiality of data, on the thesis guide, and the introduction requires faculty to ensure the subject is feasible, economical and original. That makes the guide's sign-off the first gate, not a formality. The older NBE-era submission form carried signature lines for the candidate, the guide, the head of department and the head of institution, and most hospitals still route the protocol through all four before it reaches the ethics committee. Ask your DNB coordinator what your hospital's internal route is, because NBEMS does not prescribe it.
Step 4: the Institutional Ethics Committee
Section III makes ethics committee approval mandatory before the research work begins, and makes the accredited hospital, not NBEMS, responsible for evaluating and approving the protocol through its Institutional Ethics Committee[5]. Section V sets what that committee must look like: multidisciplinary and multi-sectoral, preferably eight to fifteen members with preferably half non-affiliated or from outside the institution, a quorum of five, written standard operating procedures that may draw on the ICMR guidelines, and registration with the National Ethics Committee Registry for Biomedical and Health Research through naitik.gov.in, under the Department of Health Research notification the guidelines cite. The committee ensures scientific soundness through an appropriate scientific review committee, so expect a scientific review as well as an ethical one; the older NBE guidelines named a separate Scientific Research Committee, and many hospitals still run one.
If your hospital has no registered committee, Section V allows it to use a nearby institution's registered committee under a memorandum of understanding submitted to the NBEMS accreditation department. Find out in your first month which committee your hospital uses and when it meets. No data is collected before approval; a dataset collected before the approval date is not rescued by a letter dated after it.
Step 5: the Thesis Protocol Approval letter
Once the committee approves, the hospital, not you, submits the protocol to NBEMS in the format of the annexure titled Thesis Protocol Approval. The annexure is a letter on the hospital's official letterhead to the Executive Director of NBEMS, giving the date of the ethics committee meeting, a table of the candidates covered with specialty, session, roll number and thesis title, a statement that the committee is registered with the Department of Health Research, the authenticated composition of the committee as an enclosure, and a certificate that the protocols have not been and will not be submitted elsewhere for any degree, fellowship or title. It is signed by the academic head or DNB coordinator and by the administrative head. Read your own row in that table before it goes: the title on the letter is the title NBEMS will hold against your thesis.
Step 6: the portal
The notice of 29 November 2022 introduced the Online Thesis Protocol Submission Web Portal and required candidates who joined from the 2021 academic session onward to submit the approved protocol through it. The portal lives under natboard.edu.in/thesisonline. The index page offers a login by DNB or DrNB registration number with a one-time password, a PROTOCOL link that opens an online thesis protocol submission form asking first for your roll or registration number, and a separate PROTOCOL DEFICIENT DOCUMENT link for re-uploading a document NBEMS has flagged as deficient. The same page carries the guidelines PDF and the notice board, which is where every change to the rules has so far been announced.
Section IX of the guidelines, written for the thesis application, states the rule that also governs the protocol screens: information entered online is treated as correct and factual, NBEMS does not edit or alter it under any circumstances, and no request for change is entertained. Check the title, the specialty, the guide's name and the registration number character by character before submitting, and keep a PDF of every screen you confirm.
Step 7: the acknowledgement number
Submission generates a thesis protocol submission acknowledgement number. Section IV states the two consequences of not having one: you cannot submit a thesis without it, and a trainee who misses the protocol window is not eligible to appear in the final theory examination in which they would first have appeared. Save the number in three places, and make sure your DNB coordinator has it on file, because the online thesis application will ask for it at 26 months when the coordinator who submitted the protocol may have moved on.
Step 8: if NBEMS flags a deficiency
The PROTOCOL DEFICIENT DOCUMENT link exists because protocols are sent back for missing or defective documents. The guidelines do not publish a list of deficiencies, so none is invented here, but every document the guidelines require is a document that can be found wanting: the approval letter not on letterhead or not signed by both heads, the committee composition not enclosed, the committee not registered, the protocol outside the word limits, references not in ICMJE style, or a title that differs between the protocol and the approval letter. Fix what is named, re-upload through the deficient-document route rather than starting a fresh submission, and keep the correspondence.
Step 9: what the guidelines do not say
The guidelines are silent on changing a topic, a guide or a design after the protocol is submitted. Two things follow. First, the Thesis Protocol Approval letter certifies the title to NBEMS and the thesis layout requires the Thesis Protocol Approval to be bound in as an appendix, so a thesis that does not match its own protocol invites a question the document gives you no answer to. Second, the DrNB article-in-lieu notice of 6 August 2025 requires the article to align with the protocol submitted at the start of the course, which shows how NBEMS treats the protocol as the fixed reference. If a change becomes unavoidable, ask NBEMS in writing through the Communication Web Portal before acting, with your name, subject, registration number and date of submission, which Section XIII requires in every query.
Before you upload: a checklist from the document
- Joining date confirmed with the coordinator; protocol date and research start date calculated from it
- Title at or under 120 characters with spaces, carrying population, intervention, comparison and outcome, no abbreviations
- Each section inside its word limit; references 10 to 25 in ICMJE style
- Methods carry all ten elements, with the sample size formula, assumed values and their source
- Proforma, participant information sheet and consent form attached
- Guide, department and institution sign-offs as your hospital requires them
- Ethics committee registered with NECRBHR; approval minuted before any data is collected
- Thesis Protocol Approval on letterhead, meeting date filled, composition enclosed, both signatures present, your row correct
- Every online entry checked character by character before confirmation
- Acknowledgement number saved and shared with the coordinator
MDThesis mentors read a protocol against this list before it goes to the committee. The protocol remains yours, the committee's decision remains the committee's, and nothing here is a promise of approval.
